Community Health Service
Community Health Service
At Gulf Coast Health Center, we strive to treat the whole patient. Through our Community Health Workers program, we provide patients in our service areas advice, counseling, and other services to help them attain not only the health care services and medications they require, but also access to the additional basic resources available to those in need within our communities.
When everyone’s basic needs are met, not only individuals, but entire communities thrive. At Gulf Coast Health Center, it’s our goal not only to help our patients stay healthy by providing well-rounded, comprehensive health care but by also directing those in need to all the services and resources available to them within our communities.
What Community Health Services Provides
Gulf Coast Health Center’s Community Health Services offers our patients a single point of access to a variety of resources available to members of our community. Our representatives provide support to patients in need of housing, food, or assistance with health care costs. We connect patients to local housing assistance programs, food pantries, and the County Indigent Health Care Program.
We not only provide education on the services available to those in need, but also information regarding program requirements, locations, and deadlines. We can help you manage your requests, locate offices, make phone calls, and provide general assistance to patients in need of any other necessities or advice.
