Our Case Management Team works with patients holistically to coordinate access to resources for healthcare and social determinants of life. Support is available through navigating patients’ office visits regardless of a patient’s ability to pay. Our Case Manager acts as a liaison for patient enrollment in government funded healthcare programs and grant funded healthcare programs. Many of these funded programs cover office visit expenses and medication expenses at no cost to the patient.
